Dear patient,

We are delighted to inform our patients; Dr Tom Finnegan, Dr Peter O'Keane & Dr Gerardine O'Kelly and our Salem Medical Team are forming a new partnership with Centric Health on 1st October 2023. Salem has been a Medical Practice since 1935 and this new Partnership is to ensure continuity of care for you and your families in the years to come. The current medical team as you know it will remain in place.

Centric Health is a high quality Primary care Community with a growing network of GP Practices across Ireland since 2004.

All the staff at Salem will endeavor to assist you in any way possible throughout this transition. We have attached a frequently asked questions (FAQ) below which should answer your queries. If you have further concerns please contact Reception.

In order to comply with legislation, we must attempt to communicate with every patient for whom we have a medical record. We acknowledge that we may inadvertently contact patients who are now deceased, but whose record has not been updated on our systems. If it transpires that this has happened, we offer our deepest regret for any pain this might cause.

WHO IS CENTRIC HEALTH?

Centric Health is a high-quality Primary Care group with a growing network of family GP practices and patients across Ireland. The company was founded in 2004 by two doctors, Dr Maurice Cox and Dr Ray Power. Centric Health was established with a goal to provide healthcare in a community setting, centred on the needs of our patients. Centric Health are constantly evolving and striving to ensure that they provide world class care for more than 450,000 patients across Ireland. Centric Health are proud to say that they now have over 1000 clinical staff working to support patients need across all practices.

What does this mean for you our patient?

  • Your medical care will now be managed and maintained by the partnership of Salem and Centric Health.

  • Dr Finnegan, Dr O'Keane & Dr O'Kelly and the existing team will be delighted to continue providing medical care to you and your family.

  • Your care will continue with our team of professional Nurses and Reception Staff. There will be no reduction in services.

Do I have to move to Centric Health?

  • No, If you would prefer to transfer to another GP of your choosing, please contact the practice or complete the relevant form and we will transfer your medical records through a secure portal. 

  • IF TRANSFERRING please advise us before 1st October 2023. After this date Centric Health will become the noted Data Controller of your medical data.

Do I need to re-register if I transfer my medical files to Centric Health?

  • No, you will not need to do anything; the transition to a new partnership has no effect on your registration and your entitlement.

What happens to my data if I choose to do nothing? 

  • After the 1st October 2023, all patient records will become part of the new Salem Centric Health Partnership. You have the option at any time to transfer to another GP practice, at which time your file will be placed in an inactive status by Centric Health and will be securely transferred to a named GP.
